MassHealth dental payment posting and denials
How FirstOps posts MassHealth Dental remittances: reason codes, frequency limits, other insurance, late filing and recoupments, and how to prevent denials.
Practices with many MassHealth patients know the remittances are their own kind of work. A single MassHealth remittance can carry hundreds of lines, each with a reason code, and some payments arrive smaller than expected because MassHealth is taking back money it paid earlier. Posting them correctly takes time most front desks don't have, so the backlog builds and the denials nobody had time to read stay denied.

What we see on MassHealth remittances#
Patterns that come up again and again when we read MassHealth Dental remittances:
- Frequency limits. MassHealth pays some procedures only so often: for example, a core buildup (D2950) once per tooth every 60 months, and a limited number of periapical X-rays a year. When the limit was already used, the line is denied. Often, the earlier claim that used the benefit was the practice's own.
- Benefit plan restrictions. A procedure that isn't covered on the member's plan on that date is denied as a plan restriction.
- Other insurance. If the member has another plan that pays first, MassHealth denies the claim until the primary insurer is billed and its explanation of benefits is attached.
- Bundling. Some X-ray and exam codes are paid as part of another procedure, not on their own.
- Late filing. Claims past MassHealth's deadline are denied. When the delay came from billing the primary insurer first, the claim may qualify for MassHealth's late-filing waiver. Check before writing it off.
- Recoupments. MassHealth can recover earlier overpayments by holding back part of later payments. The balance appears on the remittance's A/R lines, so a payment can be smaller than the claims it covers.
What FirstOps does with them#
- Posts clean payments into your practice management system, usually within 24 hours of the remittance.
- Records every write-off with MassHealth's reason, so you can see which denials were avoidable.
- Flags what can still be recovered: claims to bill to the primary insurer first, late filings to check for a waiver, and denials the treating dentist may want to reconsider.
- Tracks recoupment balances from the A/R lines, so a short deposit is explained instead of a mystery.
- Sends one weekly report with all of the above.
How to stop the preventable ones#
Most frequency-limit, plan-restriction and other-insurance denials can be caught before the visit with an eligibility and history check. FirstOps runs eligibility checks too, and feeds what it sees on the remittances back into what it checks next time.

Frequently asked questions
Who administers the MassHealth Dental Program?
The MassHealth Dental Program is administered by DentaQuest on behalf of MassHealth. Providers reach it at masshealth-dental.org and at 1-866-616-2699.
Why is my MassHealth payment smaller than the claims on the remittance?
Often because of a recoupment: MassHealth is recovering an earlier overpayment by holding back part of each payment until the balance is cleared. The balance is shown on the remittance's A/R lines.
Why did MassHealth deny a claim for "other insurance"?
MassHealth pays after any other plan the member has. Bill the primary insurer first, then resubmit to MassHealth with the primary's explanation of benefits. Don't write the claim off.
Can a late MassHealth claim still be paid?
Sometimes. If the delay came from billing a primary insurer first, the claim may qualify for MassHealth's late-filing waiver. Check before writing it off.
